Hospice of Alabama offers specialized programs to meet the needs of all out patients, loved ones, and caregivers. To refer a patient to one of the programs, please reach out to your local office.
Memorable Journey
Memorable Journey is an approach to Alzheimer’s Disease and dementia care that is both patient-centered and family-focused. With the whole person in mind, care is both individualized for patients and evidence based.
Memorable Journey offers solution to the most distressing issues in care for Alzheimer’s and dementia patients. Clinicians partner with you and assist you in providing optimal care to your patients or loved ones through their hospice journey.
Any hospice patient with Alzheimer’s or dementia is eligible for this program, regardless of primary hospice diagnosis.
Benefits of Memorable Journey include:
- Personalized and individualized care for patients and their families
- Staff specially trained in Alzheimer’s and dementia care
- Team approach which addresses the physical, emotional, and spiritual needs of patients and families
- Specific protocols for management of symptoms including anxiety and agitation
- Family and caregiver support
Memorable Journey patients receive a comfort kit that includes:
- Weighted blanket (3 lbs) to provide a sense of security and comfort, which helps to reduce anxiety and agitation
- Hypo-allergenic lotion to assist with calming and agitation
- Keepsake box to store special items that assist with memory recall
- The 36-Hour Day, an education and support book for families
- CD player customized with favorite music to bring joy and comfort
- Nosey cup, a specially designed cup to aid in drinking and preventing spills

Heart Touch Journey
Heart Touch Journey is a patient and family centered clinical specialty program designed to help cardiac disease patients stay in their home setting and reduce uncomfortable and length trips to the emergency room.
Heart Touch Journey includes a specially-designed cardiac medication kit to keep in the home. Education and support are provided to the patient and caregivers regarding the medications and their usage, and guidance is offered about when to call hospice for assistance with pain management, additional support, or emergencies.
Benefits of the Heart Touch Journey include:
- Cardiac comfort kit of medications in the home to manage pain and symptoms
- Daily calls or visits if needed to reduce uncomfortable and lengthy trips to the emergency room
- 24/7 access to an on-call nurse
- Better quality of life at home through physician, emotional and spiritual support
- Reduced readmission rate
Heart Touch Journey Details:
- All hospice patients with a cardiac diagnosis are eligible for Heart Touch Journey
- There is no cost for the program under the hospice benefit. All related medications, supplies and equipment are paid for under hospice
- Education and support are provided to patients, caregivers, and families regarding medications, and 24/7 support is available

Clear Journey
Clear Journey offers education, support and treatment through a team approach to address the medical, emotional, and spiritual needs of pulmonary patients, their caregivers and their families.
Benefits of Clear Journey include:
- Pulmonary comfort kit of medications in the home to manage symptoms such as difficulty breathing, anxiety and pain
- Daily calls or visits to reduce uncomfortable and lengthy trips to the emergency room
- 24/7 access to an on-call nurse
- Better quality of life at home through physical, emotional and spiritual support
Clear Journey Details:
- All hospice patients with a pulmonary disease diagnosis are eligible for Clear Journey
- There is no cost for the program under the hospice benefit. All related medications, supplies and equipment are paid for under hospice
- Education and support are provided to patients, caregivers, and families regarding medications, and 24/7 support is available
